Opioid Treatment with Wrap-Around Care
Our outpatient opioid treatment program (OTP) fosters a culture of healing and community to help with your recovery. We provide medication assisted treatment (MAT), counseling and psychiatric services, all in one center, to support you in achieving your goals.

Whole Person Care
Foundations Health’s opioid treatment program (OTP) fosters a culture of healing and community in which individuals feel empowered to seek a new healthy lifestyle. Each patient receives an individualized treatment plan that meets their specific needs. Every person is unique, and we work with you to assess and determine the best treatment options for you.
Break the Stigma
Addiction recovery is possible, although people may put off the decision to get help for a variety of reasons. If you or a loved one is in need of help, realize there is a light at the end of the tunnel and help is available if you are ready to take action.
Foundations Health is here to help.
